Instructions & help
  1. You simply fill the personal details on page 1 & sign at all the place marked
  2. For FATCA, use one form for one applicant, if you are applying jointly provide the FATCA Form.
  3. NACH & IIN Form should be provided jointly.
  4. KYC should be provided for each individual.
  5. Attach following documents:-
  • 1 Passport Size photo.
  • PAN Card Copy (Self Attested)
  • Address Proof Copy (Self Attested)
  • Cancelled Cheque with Name ("National Securities Clearing Corporation Limited") written on it.
Note:- Please do not fill anything inside or fill exactly with reference to the attached proofs. Please issue a single cheque for all investments in the name "NSE Clearing – New Mutual Fund Platform".
